Use your legal name.
If you live in an apartment, make sure you fill in the apartment number of your residence.

Cancelling Your Voter Registration

If you've moved out of the County or State, please cancel your voter registration.

Just fill out the attached "Authorization to Cancel Registration" form and sign and date it. You may return it by mail, fax or email it to:
